The General Consent form is very important because it represents the agreement of the patient or his family to receive health services. Analysis conducted on 98 samples of General Consent forms collected between September-November 2024 showed that only 70.71% of the forms were completely filled in while 29.29% were incomplete. Important information that is not filled in completely such as date of birth address and telephone number. This incompleteness can affect the quality of medical records and risk the hospital's accreditation status. This study emphasizes that a complete form is essential to ensure that patients are informed of their rights and obligations, thereby reducing the risk of legal problems. Improvements in the process of obtaining consent are essential to improve the quality of service, ensure accurate data collection and ultimately improve patient care outcomes.
